

As part of the Open Enrollment process Crate & Barrel Holdings, Inc., evaluated not only the benefits offered to all associates, but also the platforms that help you manage those benefits. Lively has been selected to partner with CBH and provide you with a new health and lifestyle benefits platform for your Health Savings Account (HSA), Flexible Spending Accounts (FSA) and Commuter Benefits.

What to expect during transition?
If you are currently enrolling in an HSA, FSA and/or Commuter account, please find details on the transition of each benefit to Lively below.
HSA
During the week of 01/13/2025, you will be given the opportunity to opt in to a bulk HSA transfer from BRi to Lively. You will see a pop-up message in your new Lively account prompting you to consent to the bulk HSA transfer from BRi to Lively. You will have until 02/2/25 to consent to participate in the bulk transfer. If you do not consent, your HSA will remain at BRi, and your BRi debit card will continue to work as normal.
There will be a blackout period between 02/20/25 and 03/14/25 where you will not have access to your HSA funds at BRi. Your BRi debit card will be turned off on 02/17/25.
On 03/17/25 your HSA balance will be transferred to your Lively HSA.
FSA
The 2024 FSA plan year ends on 12/31/24. After that, the 60 day run out period begins and will run until 03/01/25. During the 60 day run out period, you can submit FSA claims in your BRi account that you have already incurred during the 2024 FSA plan year. Your BRi FSA card will be shut off on 12/31/24 at the end of the plan year.
After the run out period ends on 03/01/25, there will be a blackout period from 03/01/25 through 03/10/25 where you will not have access to your remaining FSA funds.
On 03/10/25 your FSA balance up to $570 will be deposited to your Lively FSA or LPFSA. Please note that the carryover is only applicable for FSA and LPFSA and not applicable for dependent care FSA.
Commuter
If you have any Commuter expenses that you have already incurred, we encourage you to submit your claims in your BRi account by 12/31/24. Your BRi commuter card will be shut off on 12/31/24.
If you have a remaining balance after 12/31/24, your balance will be moved to your new Lively Commuter account by 01/10/25. Please note that there will be a blackout period between 12/31/24 and 01/7/25 where you will not have access to your Commuter balance until the funds are transferred to your new Lively Commuter account.
When will my new account(s) be set up in Lively?
If you enrolled in the HSA, FSA and/or Commuter benefit during Open Enrollment, your new Lively account(s) will be opened for you within the first two weeks of December, 2024.
Be on the lookout for an email from Lively inviting you to complete the set-up of your new Lively account.
When will I receive my Lively debit card?
You will receive your new Lively debit card by 01/1/2025. Cards typically take about 7-10 business days to arrive from the date that they are mailed and will be mailed to you when your Lively account is opened.

Who Is Eligible?
All full time associates scheduled to work 30+ hours per week and enrolled in one of the BCBSIL High Deductible Health Plans (HSA Plan 1 or 2) AND meet the following requirements:
- Have no other health insurance coverage except what’s permitted by the IRS
- You are not enrolled in other non-HDHP medical coverage, including Medicare, Medicaid, or Tricare
- You are not claimed as a dependent on someone else’s tax return
A Health Savings Account (HSA) paired with one of the High Deductible Health Plans, will help you and your family plan, save and pay for qualified health care expenses. An HSA empowers you to build savings for health care expenses in a tax advantaged savings account that you own and control.
Four reasons to love an HSA
- Tax-free. No federal tax on contributions, or state tax in most states. Withdrawals are also tax-free as long as they’re for eligible healthcare expenses.
- No “use it or lose it.” Your balance rolls over from year to year. You own the account and can continue to use it even if you change medical plans or leave the company.
- Use it now or later. Use your HSA for healthcare expenses you have today or save it to use in the future.
- Boosts retirement savings. After you retire, you can use your HSA for healthcare expenses tax-free, or for regular living expenses, taxable but no penalties.
- Invest. HSA accounts are interest bearing and additionally you can opt to invest the funds.

- Your HSA account is set up after you enroll. (may include an ID verification step that must be completed in 30 days)
- You can contribute up to the limit set by the IRS (includes the amount that Crate & Barrel Holdings, Inc. is contributing).
- you can use money in your HSA to pay for any qualified health care expenses for you, your spouse and your tax dependents, even if they are not covered on your plan. Examples of qualified health care expenses include: your insurance plan deductibles, copayments, and coinsurance; doctor’s office visits; prescriptions; dental treatments and x-rays; and eyeglasses and vision exams.
How Much Can I Contribute To A Health Savings Account (HSA)?
Each year the IRS establishes the maximum contribution limits. The chart below represents the IRS limits for 2025. These limits are for the total funds contributed, including company contributions, your contributions and any other contributions. It is up to you as the account owner to ensure you do not exceed these IRS contribution limits annually. Please keep in mind you can change your HSA allocation at any time during the plan year.
Will CBH Contribute Money Into My Health Savings Account (HSA)?
Yes! CBH contributes the amounts listed below based on your plan election. Contributions to your HSA account will be made each pay period and are pro-rated based on coverage date.
* At age 55, an additional $1,000 contribution is allowed.
Accounts are individually owned, those under family coverage can each open their own account. Married couples cannot jointly own an HSA account, but combined cannot exceed the family limit between them.
Lively Debit Cards
Lively debit cards will be mailed no later than Tuesday, December 24th. Please note, it may take up to two weeks for delivery via USPS.
If you incur an expense before your debit card arrives, you can still get reimbursed! Follow these steps to submit your reimbursement.

HSA Loans are now available through Lane Health
As a Crate and Barrel associate, you are eligible to apply for an HSA Loan through Lane Health. You can apply and receive a minimum $500 line of credit up to $7,000 with no credit checks. You may elect multiple different repayment options. Healthcare expenses are repaid in 1 or 4 installments, and hospital expenses over 12 months. There are no annual fees, interest charges, or credit risk.

A healthcare FSA allows you to set aside tax-free money to pay for healthcare expenses you expect to have over the coming year. The limited FSA allows you to set aside tax-free money to pay for dental and vision expenses you expect to have over the coming year.
How It Works
- You estimate what you and your family’s out-of-pocket costs will be for the coming year. Think about what out-of-pocket costs you expect to have for eligible expenses such as office visits, surgery, dental and vision expenses, prescriptions, even eligible drugstore items.
- You can contribute up to $3,200, the proposed annual limit set by the IRS. Contributions are deducted from your pay pre-tax and reduce your taxable income.
- During the year, you can use your FSA debit card to pay for services and products. Withdrawals are tax-free as long as they’re for eligible healthcare expenses.
Things To Consider
- Estimate carefully. If you don’t spend all the money in your account, you can roll over up to $640 to use the following year. Any additional remaining balance will be forfeited.
- Expenses must be incurred between January 1, 2024 and December 31, 2024 and submitted no later than March 15, 2025.
- Keep your receipts as proof that your expenses were eligible for IRS purposes.
- Eligible Expenses now include more over-the-counter items.

A Flexible Spending Account (FSA) provides you the opportunity to pay for dependent care expenses on a pre-tax basis. By anticipating your family’s dependent care costs for the next plan year, you can lower your taxable income.
How It Works
Each plan year you designate an annual election to be deposited into your dependent care account. Your annual election will be divided by the number of pay periods in the plan year and deducted equally from each paycheck on a pre-tax basis. For dependent care expenses, you have access to the amount of the current contributions in your account at the time you request reimbursement.
Things To Consider
- Be conservative when estimating your annual election amount. If you don’t spend all the money in your Dependent Care FSA, any additional remaining balance will be forfeited.
- Your 2025 contributions must be used for expenses you incur January 1, 2025, to December 31, 2025, and submitted no later than March 1, 2026.
- You cannot stop or change your FSA contribution amount during the year unless you have a qualified life event.
- Expenses reimbursed through an FSA cannot be used as a deduction or credit on your federal income taxes.
- It is important to save your receipts. Lively may ask you to provide a copy to substantiate a claim. Certain claims require certain documentation.
- Reimbursements are limited to the amount contributed at the time of submission; however, you can submit your full amount of expenses and Lively will reimburse per balance and pend the rest of the eligible claim until further contributions post.
Note: Crate & Barrel Holdings, Inc. conducts nondiscrimination testing to ensure that the Dependent Care FSA does not discriminate in favor of key associates. Elections may need to be adjusted based on the test results.

Who Is Eligible?
All full time, part time and seasonal associates.
Mass Transit and Parking Expense Savings
Do you have out-of-pocket commuting expenses for public transportation, van pooling, or for worksite parking? If so, you can save on taxes by enrolling in our commuter benefit plan, administered by Benefit Resource, Inc.
The account lets you set aside money—before it's taxed—through payroll deduction. The 2025 tax-free limit is $325 per month for mass transit expenses or $325 per month for parking expenses.
Qualifying Commuting Expenses
Qualified workplace commuting expenses must be for mass transit and/or parking expenses incurred between a residence and place of employment.
- Mass transit expenses include buses, trains, subways, ferries, and vanpools.
- Parking expenses are those incurred near your workplace (e.g. SpotHero) or at a location from which you commute to work (e.g. a park-and-ride).
- Parking claims must be submitted within 180 days of the expense.
How it works
- You may enroll in or stop this program at any time. If you decide to enroll in both a mass transit and parking account, you must sign up for them separately and fund separate elections for each account.
- Your election will be deducted from your paycheck and deposited into your account.
- After initial enrollment, you will receive your benefits card in the mail to use at qualified mass transit and/ or parking vendors.
- Workplace mass transit expenses must be purchased using the card. In the unlikely event that a merchant does not accept the card for eligible parking and vanpooling expenses, claim reimbursement is available.
- Money in the account can be used in future months or plan years. Excess cash balances cannot be refunded from an account and can only be used for qualified expenses.
- A loss of benefit eligibility will affect eligible expenses and card access to account funds.
