We provide short and long-term disability benefits and a base amount of life and AD&D insurance to help you recover from financial loss.
If You Need Additional Coverage

Your Beneficiary = Who Gets Paid
If the worst happens, your beneficiary—the person (or people) on record with the life insurance carrier—receives the benefit. Make sure that you name at least one beneficiary for your life insurance benefit, and change your beneficiary as needed if your situation changes.
BASIC LIFE & AD&D
Who Is Eligible?
All full time associates scheduled to work 30+ hours per week.
Life insurance will pay your beneficiary a lump-sum payment if you should pass away while covered under the terms of this policy. The money can help your family pay for basic living expenses, final arrangements, tuition and more.
Accidental Death & Dismemberment (AD&D) insurance pays a benefit if you survive an accident but have certain serious injuries. It can pay an additional amount if you die from a covered accident.
CBH provides all eligible associates with Basic Group Life & AD&D coverage at no cost to you!
Beneficiary Designation
We recommend you designate a beneficiary for your life insurance policy(ies). A beneficiary is the person (or people, estate, trust, etc.) to whom benefits will be paid in the event of your death. You may change your beneficiary at any time during the plan year.
Age Reduction
Benefits terminate at end of employment or retirement.
Continuation of Coverage
You may be able to continue your coverage if you leave your job for any reason other than sickness, injury, or retirement.
Conversion
You can convert your group term life and AD&D coverage to an individual life and AD&D insurance policy without providing evidence of insurability if you lose coverage due to leaving your job or for another reason outlined in the plan contract.
VOLUNTARY LIFE & AD&D
Who Is Eligible?
If you are a full time associate regularly scheduled to work 30+ hours per week, you have the option to purchase additional life insurance coverage for yourself, your spouse and your dependent children. However, you may only elect coverage for your dependents if you elect additional coverage for yourself.

Initial Eligibility
When you are first offered this coverage, you can choose a coverage amount up to the Guaranteed Issue limits as outlined above without providing evidence of insurability (EOI).
Outside of initial eligibility, all amounts newly elected or any increases during this enrollment will require evidence of insurability (EOI).
Age Reduction
Coverage amounts begin to reduce at age 70 and benefits terminate at end of employment and retirement. Spouse life insurance will terminate the earlier of the associate’s end of employment/retirement or when your spouse attains age 70.
Continuation of Coverage
You may be eligible to apply for continuation of coverage should your employment end for a reason other than total disability or retirement, and the insurance has been in force for at 12 months in a row just prior to the date employment ends.
Conversion
You may have the option to convert your policy if your insurance or insurance of a dependent terminates due to termination of employment or a dependent’s ceasing to be an eligible family member due to your death or divorce, or a child’s marriage or attainment of the limiting age.
SHORT-TERM DISABILITY
Who Is Eligible?
All full time associates scheduled to work 30+ hours per week.
Many medical conditions can keep you out of work. Short-Term Disability (STD) insurance replaces part of your income for limited duration issues such as:
- Pregnancy issues and childbirth recovery
- Prolonged illness or injury
- Surgery and recovery time
Cost
CBH pays 100% of the premiums, however, any benefits you may receive will be taxable. STD payments may be reduced if you receive other benefits such as sick pay, workers' compensation, Social Security, or state disability.
LONG-TERM DISABILITY
Who Is Eligible?
All full time associates scheduled to work 30+ hours per week.
Long-Term Disability (LTD) benefits cushion the financial impact of a disability. LTD insurance replaces part of your income for longer term issues such as:
- Debilitating illness (cancer, heart disease, etc.)
- Serious injuries (accident, etc.)
- Heart attack, stroke
- Mental disorders
If you qualify, LTD benefits begin after short-term disability benefits end.
- It can protect you from having to tap into your retirement savings.
- You can use LTD benefits however you need, for housing, food, medical bills, etc.
- Benefits can last a long time―from weeks to even years―if you remain eligible.
Cost
CBH pays 100% of the premiums, however, any benefits you may receive will be taxable. Payments may be reduced by state, federal, or private disability benefits you receive while disabled.
*Pre-Existing Condition:
If you have a medical condition that begins before your coverage takes effect, and you receive treatment for this condition within 3 months leading up to your coverage start date, you may not be eligible for benefits for that condition until you have been covered by the plan for 12 months.
